2025 Fission Document Requirements

All parents/guardians must review, sign, & return ALL of the following documents to Fission Leadership PRIOR to any Athlete participation for the 2024-2025 season (including practices).

1. Fission Code of Conduct (Policy Handbook)
2. Fission Participation Agreement (Assumption of Risk, Waiver of Liability, & Indemnification Agreement)
3. VSU Participation Agreement (Assumption of Risk & Insurance Certification)
4. Fission Agreement for Minor to Travel with an Unrelated Adult (only required if the Athlete will ever ride/travel with a Fission owner, employee, or coach)
5. USAV Code of Conduct (signature not required)

If you have not done so already, please scroll below to view, download, print, sign, and deliver your completed documents to Fission Leadership ASAP. You can upload them here, email them to [email protected], or bring/mail them to our offices at 3350 Noble Way, Suite A, Valdosta, GA 31605.

IMPORTANT: All documents must be returned AND all CLUB DUES must be CURRENT PRIOR to ATHLETE participatiON in any Fission activities.

1. 2025 Fission Code of Conduct

All parents/guardians must review, sign, & return the following Code of Conduct and policy handbook to Fission Leadership prior to any Athlete participation for the 2024-2025 season.

2. 2025 Fission Participation Agreement

All parents/guardians must review, sign, & return this Assumption of Risk, Waiver of Liability, and Indemnification Agreement prior to any Athlete participation for the 2024-2025 season.

3. 2025 VSU Participation Agreement

All parents/guardians must review, sign, & return Valdosta State University’s Assumption of Risk and Insurance Certification prior to any Athlete participation for the 2024-2025 season.

4. 2025 Fission Agreement for Minor to Travel with an Unrelated Adult

Any parents/guardians that will be allowing their Minor/Athlete to ever ride/travel with a Fission Volleyball Club representative (whether an owner, employee, or coach) must review, sign, & return this document prior to such an occurrence.

5. 2025 USAV Code of Conduct

All parents/guardians must review the following document, and Athlete participation in any Fission activity is an acceptance of the standards therein.
