2025 Fission Document Requirements

All coaches must review, sign, & return ALL of the following documents to Fission Leadership PRIOR to any practices or other participation for the 2024-2025 season.

1. Fission Coaches Handbook (Policy Handbook)
2. Fission Independent Contractor Coaching Agreement (Subcontractor Relationship Agreement)
3. Fission Independent Contractor Coaching Waiver Agreement (Assumption of Risk, Waiver of Liability, and Indemnification Agreement)
4. VSU Youth Programs Staff and Volunteers Code of Conduct (Minors on Campus requirement)
5. USAV Code of Conduct (signature not required)
6. IRS Form W-9 (for tax purposes)

If you have not done so already, please scroll below to view, download, print, sign, and deliver your completed documents to Fission Leadership ASAP. You can upload them here, email them to [email protected], or bring/mail them to our offices at 3350 Noble Way, Suite A, Valdosta, GA 31605.

IMPORTANT: All documents must be returned AND all USAV/AAU requirements (memberships, background checks, SafeSport training, Impact Training) must be complete PRIOR to a coach participating in any Fission activities.

2. 2025 Fission Independent Contractor Coaching Agreement

All coaches must review, sign, & return this Agreement prior to any practices or other participation for the 2024-2025 season.

3. 2025 Fission Independent Contractor Coaching Assumption of Risk, Waiver of Liability, and Indemnification Agreement

All coaches must review, sign, & return this Assumption of Risk, Waiver of Liability, and Indemnification Agreement prior to any practice or other participation for the 2024-2025 season.

4. 2025 VSU Youth Programs Staff and Volunteers Code of Conduct

All coaches must review, sign, & return this Valdosta State University Youth Programs Staff and Volunteers Code of Conduct to Fission Leadership prior to any practices or other participation for the 2024-2025 season.

5. 2025 USAV Code of Conduct

All coaches must review the following document, and participation in any Fission activity is an acceptance of the standards therein.

6. 2025 IRS Form W-9

All coaches must fill out, sign, and return the following document prior to receiving any payment as a subcontractor of Fission Volleyball Club.
